Immunization and Health Records    

 


IMMUNIZATION AND HEALTH RECORDS...In compliance with the law of the Commonwealth Massachusetts, Newman cannot permit attendance unless it has received a certificate that the has been successfully immunized according to the following regulations:

DPT: Diphtheria, Pertussis, Tetanus: 4 doses DTap/DTP or >or equal to 3 doses of Td: plus One Td booster.

Hepatitis B: 3 doses

Polio: 4 doses

MMR: (Meases, Mumps, Rubella) 2 doses of measles, one mumps and one rubella

Varicella(Chicken Pox) Physician documented proof of the disease or one dose of the vaccine if less than 13 years of age or 2 doses of the vaccine if greater than or 13 years of age.

Tuberculosis Test. If positive mantoux test, Must present Clear Chest X-ray report.

Physical Exam within the last year.  All students need a recent physical exam on file. This exam includes an all body system review and examination by a physician. This report should address whether the student has any allergies, any past surgeries, history of migraines for example, any pre-existing conditions, and if the student is fit to engage in sports activities.


A student who has been immunized must submit a doctor's certificate in English showing the month, year, and immunization, or the occurrence of clinical disease, to avoid having to repeat the immunization.
